- Current on Flea & Tick Preventative
- Minimum Age of 10 Weeks Old (Must have all Distemper Boosters)
- Signed Pet Care Agreement
- Signed Senior Dog Waiver (For dogs 7 years & older)
- Signed Bordetella Waiver (For dogs not current on Bordetella)
